Chronic Health Care Better For People With SMI In Primary Care Medical Homes With Integrated Psychiatric Care

Among people with serious mental illness (SMI), those who received care through a specialized primary care medical home (PCMH) received more health screenings and had more improvement in care for chronic health conditions than those who received usual care. The most improvement in screening rates occurred for body mass index (BMI), cholesterol and triglycerides, and blood sugar levels. The improvement in chronic illness care occurred in activation decision support, goal-setting, counseling, and care coordination.
