CVS Health To Add Four Additional Disease Management Programs, Including Depression

On August 8, 2017, CVS Health announced plans to expand the organization’s “Transform Diabetes Care” chronic disease management program to four more disease states over the next two years. By mid-summer 2019, the program will include tracks for asthma, hypertension, hypercholesterolemia, and depression.
