About This Event
Care Coordination Coming (Or Changing) In A State Near You
What comes to mind when you think about care coordination? Do you picture social workers tirelessly connecting people to the right care across each case? Do you picture Accountable Care Organizations (ACOs) bridging the divide between different services to improve patient outcomes? Maybe you think of health homes or patient-centered medical homes (PCMH). Since 2010 and the passage of the Patient Protection and Affordable Care Act (PPACA), the number of state Medicaid programs implementing care coordination initiatives has continued to grow.1
Join OPEN MINDS and PsychU as we explore the trends in care coordination across the U.S. by state. Tracking of care coordination initiatives is essential as our speakers recognize the growth of these initiatives by state Medicaid program.1 Tune into this webinar to explore whether the past 13 years of care coordination initiatives have made progress towards their goal of improving the quality of consumer care and bending the health care cost curve.
