Patients with chronic medical conditions in addition to serious mental illness are at increased risk of hospital admission.
Transitional care support with care coordination at discharge from an acute inpatient setting to an outpatient setting appeared to be associated with a significantly lower risk of hospital readmission than usual care (hazard ratio [HR]: 0.70; 95% CI [confidence interval]: 0.61–0.80; P < 0.001).
