Outpatient mental health clinics with higher fidelity to “Zero Suicide” (ZS) practices had a 69% lower likelihood of having a suicide incident. The odds of a suicide incident at clinics with high fidelity to ZS were lower than the odds for clinics with lower ZS fidelity. Suicide-specific quality improvement policies and activities and lethal means reduction had the largest effect sizes.
